The Delta HMO. The Delta HMO is a prepaid managed care plan. Members must first elect a participating primary care dentist to receive services from. Members are able to change primary care dentists but must do so prior to receiving services by another PCD. The members will receive a schedule of benefits which list the fees for specific services; the fee is the amount in which the member has to pay for the service(s) rendered. The Delta HMO plan has no deductible and no annual maximum. This plan works in network only; if a member receives services from a nonparticipating dentist including any type of specialist without prior approval the claim will not be paid. Exceptions can only be made by obtaining prior written authorization from Delta Dental. Click here for a comprehensive list of benefits.
