Two options available to assist you with your health insurance benefits....

Option #1: The convenience of one bill and one single point of contact! As a member of the Council of Community Services you will have access to our third party administration. For a minimal fee CCSNYS can assist you in offering and managing a health insurance plan(s) for your nonprofit. Through plan administration we offer the convenience of one bill and one single point of contact for all of your insurance questions.  CCSNYS also offer administration for EPO Plans (high deductible, lower cost). Option #2: Located Outside the Capital Region, Mid-Hudson Valley or Central NY? 
Would you like to administer your own benefits, or have a plan tailored to your exact needs?
 
Please note: CCSNYS only offers administration through carriers that will issue us a group plan in a particular region. If  you do not see your region listed on the rate sheet below, or if you would like a program tailored to fit your needs that you can adminster yourself, CCSNYS can still help your nonprofit through our insurance subsidiary, Council Services Plus!

Join CCSNYS
Choose Your Plan Option
Complete the CCSNYS Member Employer Information Form
Sign the Agency Resolution  - (Board President´s Signature Required)
Work with our staff to complete the HMO Enrollment Forms
Submit payment and paperwork

CCSNYS provides administrative services through a consolidated bill and a single point of contact.
This service can be providee for health, dental and life insurance
